They matter differently, too. One wastes water quietly and costs you on a bill. One damages the building and costs you a floor. One is an inconvenience until it's the only toilet in the house. Knowing which you have determines whether this is a cheap part fitted on a scheduled visit or something that needed attention last month.

What you'll notice. Water trickling into the bowl continuously, a hiss from the tank, or the toilet refilling by itself every so often with nobody near it — the "phantom flush," which means water is escaping the tank slowly enough that the fill valve tops it up periodically.
What's usually behind it?
Why it matters more than it sounds. A running toilet is the most common cause of an unexplained water bill in any property. It's silent enough to ignore for months and it runs continuously, which adds up to a genuinely startling volume.

Why the seal at the base is the serious one. It's slow, it's hidden, and it discharges under the floor — soaking subfloor, damaging the covering, and eventually rotting the material the flange is fastened to. By the time the floor feels soft, the repair is no longer just a seal.
What you'll notice. Weak or lazy flushes, needing two flushes routinely, water rising then draining slowly, or outright blockage.
What's usually behind it.
The distinction that matters most. If other fixtures are also slow, if water rises in a shower or floor drain when the toilet is flushed, or if a nearby drain gurgles — the toilet is fine. That's a line problem, and running more water into it makes it worse. Stop and call.
If your toilet has become gradually lazier over years rather than suddenly, this is the most likely reason, and it's rarely mentioned.
Water enters the bowl through small openings around the underside of the rim — the rim jets — and, on most designs, through a larger siphon jet at the bottom. Those openings direct the water to create the swirl and the siphon that make the flush work.
In hard water areas they scale up. Mineral deposits gradually narrow and eventually block them, one at a time. Nothing has broken. The toilet is simply delivering less water, less directionally, than it was designed to — and the flush weakens so gradually that most people conclude the toilet is old and buy a new one.
How to spot it. Look under the rim with a mirror and a light. Clear openings look clear; scaled ones are crusted or closed. Another tell is water entering the bowl unevenly, or all of it appearing to come from one side.
It's frequently correctable with descaling rather than replacement — which makes it worth checking before anyone concludes the fixture is finished.
The dye test, for a running toilet. Put a few drops of food colouring into the tank, leave it thirty minutes without flushing, then look in the bowl. Colour in the bowl means water is passing the flapper — a silent leak, and almost always a cheap part. This is the single most useful thing on this page for anyone with a water bill they can't explain.
The isolation check, for a slow flush. Run every other drain in the property. If they're all clear and only this toilet is sluggish, the problem is in the fixture. If more than one is slow, or if flushing raises water elsewhere, stop testing — that's a line problem and it needs a different response.
One more thing, before you plunge. Make sure there's enough water in the bowl to cover the plunger cup, use a flange plunger rather than a cup plunger, and work with firm sustained strokes for thirty seconds rather than a few sharp pushes. Most unsuccessful plunging is too gentle and too brief.
A toilet that moves when you sit on it is not a cosmetic problem, and it's not fixed by tightening the bolts harder.
Most toilet faults are cheap parts and worth repairing. A few aren't.
Repair, almost always: flappers, fill valves, flush valves, supply lines, tank-to-bowl gaskets, wax seals, handles, and scaled jets. These are inexpensive components, and the fixture around them is usually fine.

Worth knowing when replacing: rough-in distance from the wall, bowl height, and whether the existing flange and floor need attention first. A new toilet set on a bad flange has the same problem the old one had.
